How to Access Your Medical Results
You can securely view your medical results online using your Patient ID and birth date.
- 1 Enter Your Patient ID Type your Patient ID in the “Patient ID” field.
- 2 Enter Your Birth Date Enter your birth date as the password in MMDDYYYY format. Example: May 21, 1990 → 05211990
- 3 Click Sign In Click the “Sign In” button to access your medical results.
Your Privacy Matters
Your information is secure and confidential. Do not share your Patient ID or password.
