ADMISSION OF PATIENT WITH HMO CARD / COMPANY

  1. Patient arrives at Emergency Room.
  2. ER Nurse on Duty will give the admission slip to Admitting Clerk.
  3. Admitting Clerk will explain and give Admission Record to the patient or relative.
  4. Patient / relative will fill up the admission data sheet.
  5. Admitting Clerk will copy the HMO card / Company at the HMO logbook for verification.
  6. Admitting will explain and inform the patient / relative that they must submit the Letter of Authorization (LOA) within 24 hours.
  7. Admitting Clerk will interview the patient / relative and encode the data in the computer system.
  8. Admitting Clerk will ask the patient what room he/she wants to occupy and will inform the Nurse of the Nurse Station concerned the room to be prepared.
  9. If the Room and Board is not available on that time, Admitting Clerk will give a room unavailability form to the patient / relative.
  10. Admitting Clerk will explain excess and incremental for these patients with cardholders / relatives when they wanted to go to the higher than their Room and Board plan.
  11. Admitting Clerk will give the PhilHealth Eligibility Form.
  12. Patient / relative will fill up the form and forward it to the Benefit Claims Section.
  13. If the patient is not applicable to Philhealth or Non-Philhealth qualified dependent, the Admitting Clerk will inform them  that they will have to pay for the Philhealth or Self-Pay, until verification from HMO announces coverage.
  14. All vehicular accident will be admitted as Philhealth or Self-Pay until with ‘POLICE REPORT’ and other evaluation and announces coverage.
  15. Admitting Clerk will print the admission data sheet and will give it to the ER admitting Nurse on Duty.

ADMISSION WITH SENIOR CITIZEN

  1. Patient arrives at Emergency Room.
  2. ER Nurse on Duty will get the admission order from the patients / relatives that indicate the medical diagnosis, attending and admitting doctors of the patient.
  3. ER Nurse on Duty will give the admission slip to Admitting Clerk.
  4. Admitting Clerk will explain and give Admission Record to the patient or relative.
  5. Patient / relative will fill up the admission data sheet.
  6. Admitting Clerk will interview the patient / relative and encode the data in the computer system.
  7. Admitting Clerk will ask the patient what room he/she wants to occupy and will inform the Nurse of the Nurse Station concerned the room to be prepared.
  8. If with Senior Citizen card, Admitting Clerk will encode the Senior Citizen discount scheme  in the system.
  9. If without Senior Citizen card, Admitting Clerk will explain to patient / relative that Senior Citizen  requirements should be submitted to Billing Office.
  10. Admitting Clerk will print the Admission Data Sheet and will give it to the ER admitting Nurse on Duty.
  11. The ER admitting Nurse on Duty  will carry out the admitting order and will accompany the patient to the room and endorse it to the Nurse-on-Duty at the Nurse Station.

 

ADMISSION SELF-PAY WITH PHILHEALTH

  1. Patient arrives at Emergency Room.
  2. ER Nurse on Duty will get the admission order from the patients / relatives that indicate the medical diagnosis, attending and admitting doctors of the patient.
  3. ER Nurse on Duty will inform the Nurse Supervisor, patient for priming.
  4. Nurse Supervisor will explain to patient / relative to hospital policies and possible hospital expenses.
  5. ER Nurse on Duty will give the admission slip to Admitting Clerk.
  6. Admitting Clerk will explain and give Admission Record to the patient / relative.
  7. Patient / relative will fill up the admission data sheet.
  8. Admitting Clerk will give the PhilHealth Eligibility Form and let the patient / relative to fill up the form and forward it to the Benefit Claims Section.
  9. Admitting Clerk will interview the patient / relative and encode the data in the computer system.
  10. Admitting Clerk will ask the patient what room he/she wants to occupy and will inform the Nurse of the Nurse Station concerned the room to be prepared.
  11. Admitting Clerk will print the admission data sheet and will give it to the ER admitting Nurse on Duty.
  12. The ER admitting Nurse-on-Duty  will carry out the admitting order and will accompany the patient to the room and endorse it to the Nurse on Duty at the Nurse Station.
